A quest towards curing childhood cancer as well as pinpointing better molecular biomarkers

In this week's Globe Magazine cover story, reporter Mike Damiano takes readers inside the scandal that ended with broken careers, betrayed patients, and contributed to a lasting legacy of doubt in scientific institutions.

Circulating tumor DNA methylation detection as biomarker and its application in tumor liquid biopsy: advances and challenges
Based on published studies and future trials, clinical implementation of liquid biopsies in pediatric solid tumors is anticipated for several applications. Firstly, in a potential cancer screening setting for early diagnosis. However, this seems dubious for the general pediatric population and will likely only be relevant for specific risk groups, such as children with genetic predisposition. More relevant is risk-stratification following diagnosis and subsequent guidance of treatment management

By focusing on ribonucleotide reductase (RnR)—a pivotal enzyme in DNA synthesis and in cancer growth—these and other aspects of the disease are demystified. For example, this viewpoint reveals the proximate cause of uncontrolled cell growth: the hyperactivity of RnR, as confirmed by studies of retinoblastoma and HPV-related cervical cancers. This perspective also suggests a way to defeat cancer, as the free radical—essential for the activity of RnR—can be quenched by a stream of electrons.

Overall, our results demonstrate that rare germline SVs that span coding and noncoding loci are important risk factors for pediatric solid tumors. We observed stronger SV effects in neuroblastoma relative to Ewing sarcoma, which is consistent with the markedly earlier age of onset in neuroblastoma. We estimate that rare SVs contribute 1.1 to 5.6% of liability for these cancers, which underscores a modest yet meaningful role for rare germline SVs in the missing heritability of pediatric solid tumors. Our work supports the integration of germline SVs into clinical practice and research efforts related to pediatric solid tumor predisposition.

Physicians usually recommend proton radiation therapy to treat solid tumors in children because it sends less radiation to healthy tissue than standard radiation therapy does.
Less radiation means fewer short- and long-term side effects and lower risk for serious problems that may result from treatment. Studies show that proton therapy may help protect children from development problems, growth delays, lower IQ, cancer forming in other parts of their body and other health problems, compared to standard X-ray radiation.

Statistical Methods in Childhood Cancer Research

The Childhood Cancer Data Initiative: Using the Power of Data to Learn From and Improve Outcomes for Every Child and Young Adult With Pediatric Cancer

Each year in the U.S. there are an estimated 15,780 children between the ages of birth and 19 years of age who are diagnosed with cancer. Approximately 1 in 285 children in the U.S. will be diagnosed with cancer before their 20th birthday. Globally there are more than 300,000 children diagnosed with cancer each year. Every 3 minutes, somewhere in the world a family hears the devastating words that their child has been diagnosed with cancer.
